On-Site Services Aligned’s acute and post-acute coverage service uses a coordinated care approach to manage acute discharges to post-acute and ensure hospital readmissions are minimized. • Manage inpatient units at general acute care hospitals or psychiatric hospitals. • Provide psych consultation services to Med/Surg patients. Acute • Coverage of Emergency Department. Care • On-site during daytime and call coverage at night (could also use telepsych coverage for nights). Hospitalist • Staffed by physicians. • Requires professional services contract with facility and financial support stipend. Post- • Provide psych consults for post-acute patient at SNF, Transitional Care and Assisted Living facilities. Acute • Primarily staffed with psych ARNP’s with physician oversight. • Professional services contract with facility is not required and Coverage there is no financial support stipend for basic services. 5
