Intake Data 27 adults, 155 children 56% Black, 35% White, 13% Hispanic Equal numbers of male and female participants Almost 70% of MATCH client households earned below $25,000/year Enrollment and Retention 182 enrolled 173: ≥ 1 follow-up visits paired with intake 132: ≥ 6 visits in ≥ 5 months
Results Of 132 with ≥ 6 visits in ≥ 5 months 83% decrease in hospitalizations 60% decrease in ED visits 71% decrease in missed days of school 78% decrease in missed days of work 83% decrease in nights awake from asthma 145% increase in asthma action plans 30% increase in ACT scores
