Intake Data  27 adults, 155 children  56% Black, 35% White, 13% Hispanic  Equal numbers of male and female participants  Almost 70% of MATCH client households earned below $25,000/year Enrollment and Retention 182 enrolled 173: ≥ 1 follow-up visits paired with intake 132: ≥ 6 visits in ≥ 5 months

Results Of 132 with ≥ 6 visits in ≥ 5 months  83% decrease in hospitalizations  60% decrease in ED visits  71% decrease in missed days of school  78% decrease in missed days of work  83% decrease in nights awake from asthma  145% increase in asthma action plans  30% increase in ACT scores
