Involving patients and local communities at King’s We involve patients and local people to give us advice on service developments: • King’s membership for individual local people and patients • Associate membership for voluntary and community organisations • Opportunities to participate in discussions, workshops and events • Recent projects in cancer care, children and young peoples services, outpatients development We listen to patient experience to celebrate what is working and where we can improve: • Patient experience surveys for inpatients and outpatients – ‘How Are We Doing’ • Patient stories at King’s Board • Understanding patient experience across hospital, primary care and social care eg older people leaving A&E 3
How King’s works with GPs Communication • Out patient letters aim to be emailed to GP Education / Corporate within 5 working days of clinic appointment Communications : • GP referrals to hospital should come via ERS • Regular GP education events not by post or fax • 6 King’s newletters per year – • Discharge notifications via email (48 hours) service updates, education events, • Local Care Record – GPs can view their patient news about Kings record at the hospital and Hospital Clinicians • can view the patient GP record Messages from King’s via the CCG • Day to day GP queries e.g. Chasing an GP weekly bulletin appointment Quality alert/Reverse quality • GP website – referral guidance alert process; reported through Clinical Quality Improvement Group How King’s works with GPs Specialist care in the community Discharge: • Discharge Notifications sent via • Integrated Heart failure and Respiratory nhsnet secure email ( eDN’s ) into GP teams • surgery dedicated email box within Virtual clinics (diabetes, respiratory, Renal in discussion) 48 hours of discharge. • Community Multidisciplinary Team meetings • Redesigned discharge notification for people with complex issues (RCP guidance) with GPs and hospital • Care Home Interface doctors include advice for patients • Outreach clinics (paediatrics ) and actions for GP • Advice and Guidance, ‘Consultant Connect’, • Patients are given a copy/sent a copy Telephone hotlines • LTC dedicated email for GPs to ask for help by post. with care planning • Audits about the quality of eDN’s
