  6. Adolescent Immunizations Definitions 1. Percentage of unduplicated S BHC users who have received Meningococcal vaccine by their 13th birthday. 1. Percentage of unduplicated S BHC users who have received Tdap or Td by their 13th birthday. 1. Percentage of unduplicated S BHC users who have received three doses of HPV by their 15th birthday (with appropriate spacing between vaccines).

  7. Adolescent Immunizations Reporting the Measure These immunization measures will be reported on by Apex through an agreement with the Colorado Immunization Information System (CIIS).
