Call/ Recall Invite child for vaccination as per schedule – -1 mth, 2 mths and 12 mths & at 3yrs 4 mths – 5yrs (with pre school booster) place alerts for subsequent vaccinations • Ensure vaccine available Book interpreter if required (CityCare) Inform HV of appointment date Contact parent/guardian by phone or text prior to appointment to act as a reminder Actively follow up children who fail to attend clinic for imms by contacting parent/guardian NHS | Presentation to Nottingham City PNs [17 th & 24 th June 2014] Maintained a register of babies at risk of Hep B within the practice 4
Appointment/ Data Collection Inform parent/guardian of need for vaccination and schedule Vaccination should be given under a PSD Inform CHRD (SystmOne/ Monthly computer print outs) – include date, batch no, name of vaccine, expire date & site given. Monitoring and review Babies at risk of Hep B should be discussed at monthly health visitor/practice meetings, this should be attended by the lead PN or minutes/ actions fed back HV & GP will ensure where appropriate vaccination is recorded as part of child protection plans Liaise when a child fails to attend for schedule vaccination, and where appropriate offer outreach vaccinations NHS | Presentation to Nottingham City PNs [17 th & 24 th June 2014] 6
Completion On completion of the 4 th dose, arrange for the child to have surface antigen blood test (HBs Ag) at 12 months to exclude infection Any medical concerns or where a child test positive for infection should be referred to Paediatrician NHS | Presentation to Nottingham City PNs [17 th & 24 th June 2014] 7
