STUDENT HEALTH INFORMATION FORMS • ADD/ADHD-medications at home what are they? • Allergies-Do they require an Epipen? • Asthma-Do they need a rescue inhaler at school or one to carry themselves? • Seizures-even history of, when was last one, what type? • Migraines-How frequent? • Concussion-Has there been one in the last year? • Mental health-Anxiety, depression, bipolar? Are there medications?

STUDENT HEALTH INFORMATION FORMS CONTINUES • Please list medications-this helps for reactions, history for an accident, and help to see if there may need to be a medication change • List medical provider-again important if there is an emergency • Release of information-helps with communication with doctor’s office to ensure that student has everything that they need, doctor’s notes for gym, medication form clarification, restriction clarification, information for 504/IEP plans

STUDENT SELF CARRYING MEDICATIONS • Non-prescription – Note from parent or guardian that includes : • Name of medication • Date • Time medication is to be administered or how often (at lunch or every 4 hours as needed for headache) • The dosage of the medication (500mg) – Medicine should be in original container – Container should only have enough for 1-2 days – Note needs to stay with medication at all times

STUDENT SELF CARRYING MEDICATIONS • For students that are carrying prescription medication to self administer – Medication Authorization for student on record in nurses office sign by medical provider – Non expired prescription – Original labeled container (ask pharmacy for label for inhaler) – Only enough medication for 1-2 days (ie: migraine medication) – Medications should be easily accessed by student • If need inhaler make sure in gym bag • If carry epipen don’t leave in locker during lunch

  8. SCHOOL DRUG POLICY • Policy Code: 4325 Drugs and Alcohol Unauthorized or illegal drugs and alcohol are a threat to safe and orderly schools and will not be tolerated. Students are prohibited from possessing, using, transmitting, selling or being under the influence of any of the following substances: – 1. narcotic drugs; – 2. hallucinogenic drugs; – 3. amphetamines; – 4. barbiturates; – 5. marijuana or any other controlled substance listed in Chapter 90, Article 5 of the NC General Statutes; – 6. cannabinoid (CBD), or any substance containing CBD, including CBD oils; – 7. synthetic stimulants, such as MDPV and mephedrone (e.g., “bath salts”), and synthetic cannabinoids ( e.g.,“Black Diamond”, “Black Magic” “Spice,” “K2”); – 8. any alcoholic beverage, malt beverage or fortified wine or other intoxicating liquor; or – 9. any chemicals, substances, or products procured or used with the intention of bringing about or mimicking a state of exhilaration or euphoria or of otherwise altering the student’s mood or behavior. – 10. Students are prohibited from possessing, using, transmitting or selling or counterfeit (fake) drugs.

  9. ILLNESS AND ABSENCES GOING HOME SICK • If a student has had a fever (100.4 or greater) must be fever free (without medication) for 24 hours • If student has been vomiting or diarrhea must stay home 24 hours after stopping • If student is sick at school needs to come through the nurse to go home, if student calls from cell or texts will not be excused absence if they go home • Need to make sure that information that we have on record is accurate, when phone numbers change we need the updated information especially for an emergency situation • Please keep student home 24 hours after start of antibiotic

  11. 1. HAVE A POSITIVE ATTITUDE Y our attitude about school as a parent influences how your child feels about education. Be supportive of formal education. Show you are interested in your child’s academic progress. Be supportive of the teachers and administrators.

AVOID THESE NEGATIVE COMMENTS TO YOUR CHILD ▪ I didn’t do well in that subject when I was in school. ▪ Just do the work even though you will never need to know that in life. ▪ I heard from other parents their child is not doing well in that class. ▪ That teacher is hard, your older sibling didn’t do good in his/her class. *Please be supportive of your child school. If concerns come up, please direct those issues to the proper school personnel.
