DIRECTIVE N O : D-P4 070101 2 Revision: 03 I NCIDENT R EPORTING R EQUIREMENTS WITH R ESPECT TO A MUSEMENT Devices (I NCLUDING W ATER S LIDES) This Directive supersedes Directive D-P4 040730 4 issued on July 30, 2004. This Directive is being issued by a provincial safety manager pursuant to section 30 of the Safety Standards Act. Date of Issue: December 15, 2009 General Details This Directive is being issued to businesses and contractors to clarify the reporting of incidents. It is issued with respect to the requirements of section 36 and 37 of the Safety Standards Act and section 34 of the Safety Standards General Regulation. Specific Details A. Reporting of Incidents to the British Columbia Safety Authority Persons in charge of regulated work, owners of regulated products, permit holders, persons who perform regulated work, and persons who operate a regulated product or system are required to report incidents. Incident reports must be made to the British Columbia Safety Authority (BCSA) within 24 hours of occurrence. The notification and shut down criteria below should be followed when a Type I, II or III incident occurs. Incidents can be reported as follows: Telephone Contact All incidents must be reported by phone to the appropriate BCSA Safety Officer. Types Of Incidents Incidents range in severity from major (Type I) to minor (Type III). For the purposes of this Directive, incidents are categorized as follows: Type I – Must be Reported by Telephone Notification Immediately � manual evacuation except where a power failure has occurred � fire involving the equipment or structures; � failure of any electrical or mechanical component which results in the loss of control such as failure of slow or stop commands, acceleration beyond design rates, self starts, overspeed and unintentional reversal; � failure of any safety device and control to function as intended; � failure of any structural component such as a tower, shafts, loading decks, stairs, conveyance, passenger restraints, flumes or other assembly components; � explosions, � all types of collisions � derailments or fatigue failure � a loss of life or life threatening injury as a result of the use or operation of an amusement device. Type II – Must be Reported by Telephone Notification Immediately Type II incidents are any type of incident where professional medical attention is involved for any injury as a result of the use or operation of an amusement device. Type III – Must be Reported within 24 hours by Telephone � evacuation where a power failure has occurred; � falls from passenger carrying units (PCUs) outside the loading or unloading area C. Installation shut down (closed) requirements following an incident. In accordance with the authority granted to a provincial safety manager in section 20 of the Elevating Devices Safety Regulation, it is directed that when a Type I amusement ride or incident occurs, the amusement device involved in the incident must be evacuated and shut down (closed) until permission to operate (open) has been granted by a Safety Officer. It is further directed that when a Type II or Type III amusement device incident occurs, the carriage of traffic on the amusement device involved in the incident may continue unless a Type I incident is also involved.
