Community Health Services 2 Community Health Services (CHS) provides public health coverage and epidemiology activities in 12 of Nevada’s rural and frontier counties including: Lyon, Churchill, Mineral, Humboldt, Pershing, White Pine, Nye, Lincoln, Elko, Esmerelda, Lander and Eureka. CHS has 9 full time Registered Nurses (RN) and 2 full time Advanced Practice Registered Nurses (APRN) serving low income, underinsured and uninsured populations in underserved geographic areas of Nevada. Community Health Nursing staff identify needs and work to fill health care gaps in their communities that typically lack adequate medical resources. 1/24/2019
Community Health Services 3 Demographics, economic structure, and social characteristics of rural and frontier Nevada produce challenges in health care priorities, access to care and timeliness. Rural and frontier communities incur barriers to healthcare access including: limited or no available resources, a shortage in qualified and license providers, no availability or primary or specialty care, lower rates of health care coverage, long distance travel for care or inadequate public transportation. For many communities, CHS clinics are the only means to access healthcare and meet their needs. 1/24/2019
Community Health Services 4 There are 12 Community Health Service Clinics in eight rural and frontier counites: Lyon County- Dayton, Fernley, Silver Springs, Yerington • Churchill County – Fallon • Mineral County – Hawthorne • Pershing County – Lovelock • Humboldt County – Winnemucca • White Pine – Ely • Lincoln County – Panaca • WIC Services 1-2 times monthly in Alamo • Nye County – Pahrump, Tonopah • 1/24/2019
Community Health Services 5 Mandatory Services Include: Investigation and reporting infectious disease STI: control, prevention, and treatment HIV: testing, counseling referral for treatment TB: screening, control, prevention, and treatment Vaccine clinics scheduled as necessary for outbreaks Participation in Local Emergency Preparedness Committees, Points of Dispensing exercises, and Board of Health meetings 1/24/2019

Community Health Services 7 Maternal and Child Health Program Overview: MCH provides funding to CHN clinics to provide education, screenings, contraceptive care, immunizations and referrals for populations including: Women (15-44 years old), Infants, Children, Adolescents and Children and Youth with • Special Health Care Needs Scope of Work Planning, managing, and directing program activities for the benefit of MCH • populations. CHS to submit reports, documents, monitor program expenditures and perform required • program activities. 1/24/2019
Community Health Services 8 Maternal and Child Health Program funds: Approved Budget: $196,638.00 Annual Personnel Costs at 10% for clinic RNs and APRNs In-State Travel for RNs and APRNs for travel between clinics Medical Supplies: immunizations, contraceptive supplies, testing supplies, infection testing and control products, exam room supplies Operating Supplies: office supplies, outside printing, postage costs for shipping supplies to rural clinics Contractual Costs: 25% of the costs for a contracted part-time RN in Ely Clinic printing costs at 10%, communication cost at 10%, and operating lease expense for clinic copiers at 10% 1/24/2019
