Overview In 2008, the Norfolk and Waveney Mental Health Trust entered into a section 75 agreement with Norfolk County Council for the delivery of mental health social work services to working age adults in Norfolk. • This agreement ran until 31 March 2013 • 12 month extension agreed • January 2014 agreement for transition

  3. What changes are being planned? Mental Health Social Workers will: • Continue to support service users with social care needs • Work in 5 locality teams across Norfolk • Be based within the Trust in existing locations • But be employed by Norfolk County Council

  4. What will this mean to you? • Give a stronger focus on social care needs • More emphasis on personalisation and personal budgets • Increased focus on needs of carers • More work to avoid need for residential care • Greater focus on safeguarding

  5. Benefits to you • Local decision-making on care and support • More assessments and support to carers • Timeliness in drawing up support plans and personal budgets • Reviews kept up to date

  6. Each locality team will continue to have : • Team Managers • Practice Consultants • Social Workers • Assistant Practitioners • Carers assessors • Business/admin support

  7. What will stay the same for you • Your personal budget • Your current care and support • Support with managing your health and your medication • Your usual appointment with lead clinician or psychiatrist • Service user and carer support groups

Things that might change for service users and carers? • Your care coordinator – but there will be further discussion with you if this is likely to happen • Where your key worker is based • In some cases you will have two workers (health and social care) • New social care referrals through central Norfolk County Council number

Changing your care coordinator - what we will take into account • Is major focus of your support health or social care? • Do you have a personal budget? • Are you subject to Section 117 aftercare? • When is your next review due? • Impact of a change of coordinator on your recovery? • Anything else?

  10. Approved Mental Health Professional Service (AMHP) • A countywide service • Dedicated Team Manager • Full time Practice Consultants • AMHPs on duty throughout the day • Better links with out of hours (EDT) & CRHT • Improved availability and reduced waiting • Improved links with Police and Ambulance Service.
