REGISTER • Select the “REGISTER” button to register for an account. • The Registration Process requires completion of the following fields: • User selected Username • Email address • First Name, Last Name, Organization Name and Telephone Number • Selection of an Association – User selects the IDN they are associated with or State User if applicable • Enter the CAPTCHA displayed • Select the “Create new account” button 7

  8. REGISTER Note: • A Captcha entry guards against system-generated “Internet bots.” • If you do not receive an email at the address provided within 72 hours, check your SPAM folder. If an email is still not located, contact nhdsrip@mslc.com with the subject header “Waiting on Registration” to inform website support. 8

REGISTER Known Registrant? • Once the “Create new account” button is Known selected, the registration applicant will receive Registrant? an email informing them that the account is pending approval .   10. HOME • Selecting the HOME tab returns the User to the CPAS Homepage. • Hovering over HOME offers the User the ability to view and select sub-tabs that directly access important DSRIP websites, including the State Home page, State DSRIP page and IDN pages. 10

  11. CALENDARS • Selecting the CALENDARS tab allows the User to access all calendar events that the User has access to. • Hovering over CALENDARS allows the User to view and select the PUBLIC CALENDAR sub-tab. 11

  12. CALENDARS • Public Calendar Users can scroll through months for future or past events. Events will include: o All DHHS DSRIP meetings o All Learning Collaborative events • Public Calendar Events will include Event Title, Date, Time. • Myers and Stauffer can Add, Edit and Delete Events. 12

  13. PROJECT RESOURCES • Selecting the PROJECT RESOURCES tab allows the User to access key DSRIP documents: o Special Terms and Conditions (STCs) o Protocols o Project and Metrics Specification Guide o IDN Regional information 13

LEARNING COLLABORATIVE • Selecting the LEARNING COLLABORATIVE tab allows the User to access information About: o What is a Learning Collaborative? o What are the goals of the New Hampshire DSRIP Learning Collaborative? o Who is participating? • Hovering over LEARNING COLLABORATIVE offers the User the ability to view and select sub-tabs that directly access the three Learning Collaborative formats. • All IDN Administrative Lead Meetings • All Partner Statewide Meetings • A1, A2, and B1 Project Implementation Meetings 14

LEARNING COLLABORATIVE • Users may learn about the purpose, goals and meeting participants for each meeting format. • Users may access information about up- coming Learning Collaborative events. • Users may access agendas, presentations and materials after a Learning Collaborative event has been held. 15

  17. FAQ • IDN or the public can submit general questions, questions related to the Learning Collaborative, or questions related to the website. • Myers and Stauffer will respond to each submitter. • Frequently Asked Questions will be posted for Users to refer to. 17

  19. FORUMS • Selecting the FORUMS tab allows the User to access discussion forums: o General discussion o Project specific forums • Hovering over FORUMS offers the User the ability to view and select sub-tabs that directly access forums available. 19

  20. FORUMS • Select “ New Topic” to begin a new discussion. A User may sort on discussions. 20

  21. PROJECT RESOURCES • Selecting the PROJECT RESOURCES tab while logged in allows the User to access sub-tabs based on the User profile. o Technical Assistance Directory o IDN specific Resource Files 21

LC MATRIX REVIEW • MSLC follow up on current IDN requests, receives new IDN needs • MSLC to facilitate discussions related to project-specific targeted interventions, best practice, success, challenges related to statewide and community projects • Each discussion includes how project interventions intersect and relate to core competency goals • Supports evolution to project implementation • Cross-regional learning • MSLC to report promising practices to state and all IDNs (e.g. policies and APMs, etc.) 29

AUGUST 28 TH LC: ATTENDEES & PURPOSE • Planning for approximately 150 attendees (20 per IDN). • Purpose is to educate participants about strategies for practical framework to the operationalizing of integration processes and tools to achieve the IDN’s project goals. • Targeting IDN Administrative Leads, IDN Project Management staff, partner representation from project B1, and MCO representatives. 33

AUGUST 28 TH LC: LEARNING GOALS • Attendees should learn of examples of practical implementation activities and best practices that have been achieved nationally and statewide for the following: • Defining staff roles and workflows; working effectively in teams within and across disciplines, recognizing the strengths and skills of colleagues. • Defining and implementing tools and required infrastructure. How electronic tools can improve your integrated care model and strategies to address challenges with their usage. • Identifying how integration impacts your office and next steps for operationalizing required functions in partnership with participating groups keeping change management principles in mind. • Use in implementation plans and demonstrating progress as required for the July – December Reporting Period along SAMHSA Framework for Integrated Levels of Care by meeting the following additional milestones. 34

  35. AUGUST 28 TH LC: LEARNING OBJECTIVES • Apply information learned and through months of implementation work for completion of group exercise. Use the opportunity to share information from implementation plans that may provide ideas for other IDNs to consider for incorporation into their implementations. • Leave with ideas from presentations and group exercise that may benefit achievement of your IDN’s implementation plan and that you will share with the appropriate committee or individual(s). 35
